Provided you reside in our practice boundary you can register at our practice. All patients making a request to join our practice must do so by requesting an application form from our reception team. You will need to bring in proof of your address to facilitate completion of your registration in the form of utility bill/bank statement/council tax bill. All new patients are registered with the practice and not an individual partner. If the doctor of your choice is unavailable we will offer you an appointment with another GP. If a patient insists on waiting to see a specific doctor then this may result in waiting for more than 48 hours.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
You need to come into our reception to collect a new patient registration pack before you can make any appointments at the practice.
Change of Address
Please notify us if you change your address to ensure that your computer records are kept updated. If you move outside the practice area you will need to register with another GP. In order to change an address patients will have to bring in a utility bill/bank statement/council tax bill, as form of proof for their new address.
